Our programs provide direct access to care coordination.We support adults, the elderly population, and their families in developing team based guidance and services that focus on continuum of care.  We are there in the community to advocate for clients, and to help connect a wide range of community resources available such as other healthcare providers, specialty providers, and government programs.
